Horizon House has an immediate opening for an experienced Clinical Support Specialist. This position will be responsible for the following duties:
To write, implement and monitor the comprehensive service plan for young adults. Provide systematic and individualized case management and program planning incorporating psycho-educational rehabilitation approaches in compliance with the Agency's overall purpose and the individual program goals and objectives.
To provide on-going evaluation to monitor the necessary level of services required to promote an individual's growth and skill development.
To develop positive support strategies, plans, and data analysis for people with challenging behaviors.
To provide and identify counseling and training to staff in areas related to the individual’s plans.
May co-facilitate groups and therapy sessions.
Ensure compliance with all funding or licensing regulations and agency policy and procedures.
Qualifications
M.A. Degree in Human Services/Social Sciences/Healthcare or a related field and at least 1 year of experience in the MR field required; 2 years preferred; 3 years in the MH field and behavior analysis and modification experience.
Very good communication skills; prior experience in writing programming goals and behavioral plans.
Previous clinical experience conducting group and/or individual therapy sessions required.
Strong analytical and program development skills.
Ability to work independently.
Ability to respond to crisis situations.
Detail oriented and strong organizational skills.
Experience writing functional analyses and interpret data.
Ability to use word processing systems; touch typing at 45 wpm.
